Build portable Responses-style output items for pending interrupts.
Unlike :func:emit_interrupts, this helper does not require a Responses
event stream, so generic protocol adapters can expose the same HITL wire
shapes. Item IDs are deterministic to remain stable across polling and
process recovery.
