Reads managed config merged over config.toml, so a managed [effort]
still applies when the user file is unusable.
Model in provider:model format.
Path to config file.
Defaults to ~/.deepagents/config.toml. Passing a path also excludes
managed policy from this read, so production callers must pass
None.
