Resolve the effective configured and runtime model kwargs.
This mirrors the ordering used when constructing a model: provider and
per-model params, then the resolved base_url, then runtime
overrides. Consumers that need to reason about an actual request (such
as cache policy checks) must use this rather than inspecting one source
of settings in isolation.
Per-request params, which take highest precedence.
