Open model-attempt lifecycle scopes, keyed by stream namespace.
Populated by model_attempt start events and resolved by complete/retry
events. While the current namespace holds one, its
(namespace, call_id, attempt) triple scopes usage accounting so a retry
reusing the provider's message ID cannot revise the failed request; a
namespace without one records legacy unscoped (attempt_scope=None).
